Rates & Insurance

Art Class

Although CFH Art Therapy & Counseling LLC does not work directly with insurance companies, we are considered out-of-network providers to some insurance companies and our services may therefore be covered in full or in part by your health insurance plan. Each program and plan is different, so we recommend that you contact your insurance provider directly with any questions regarding coverage. We will provide you with the proper diagnostic and service codes so that you can file insurance claims on your own. Please let us before your first session if you plan to request reimbursement from your insurance company.

We understand that things come up and that you may not be able to make it to your scheduled appointment- that is okay. We ask, however, that you provide your therapist with 24 hours’ notice to avoid being billed the full cost of the session.

GOOD FAITH ESTIMATE NOTICE

You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.

You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
